Some important information about physicals, medications & dental exams for students entering the 6th & 9th grade.
Physicals
Your son/daughter will need to have a physical exam & required immunizations to enter the 6th & 9th grade. This is a state requirement & athletic physicals do not meet this requirement. This physical will however, meet the athletic physical requirement.
***The majority of physical forms have a health history section that needs to be filled out by the parent & signed. If the health history has been filled in electronically there will still be a place for the parent to sign. Please make sure you have completed your portion before turning it into the school. If your form is incomplete it will be returned to you & counted as incomplete until these state requirements are met.
Dental
Your student is also required to turn in a dental exam. This must be dated after November 2018. Please turn this in when you turn in your child’s physical. This is a new requirement for 9th grade students effective this school year.
Medications
If you think that at any point in the 2019-20 school year that your son/daughter may require Acetaminophen (Tylenol), Ibuprofen (Advil, Aleve), Cough medicine, Midol, Lactaid etc… you must have the medication authorization form filled out by your child’s physician. They can fax it to the school at 794-3351 or it can be turned in with the physical & dental forms.
You will provide the above medication(s) in the original container. Please make sure your child’s name is written on the bottle & that the medication is turned into the office.
There are no changes to the inhaler and prescription medication policies.
