Full-Time Remote Learning Requests

In order to be considered for remote instruction, this form must be submitted on or before August 12, 2021.

In light of the COVID-19 pandemic, Maroa-Forsyth CUSD #2 is developing a full-time remote learning option for students with health conditions that cause them to be at higher risk of severe illness if exposed to COVID-19, and/or for students who live with an individual who has a health condition causing them to be at higher risk of severe illness if exposed to COVID-19. Information about the districts full-time remote learning plan will be shared on, or before August 9, 2021. Parents/guardians may access that plan on the District's website at www.mfschools.net.

The District will consider requests for full-time remote instruction on a case-by-case basis. Any instructional days for approved full-time remote learning instruction will count as regular attendance days for the student. Please submit this request to the school where your student is currently enrolled. One form should be completed for each student.

All approved request for full-time remote instruction will apply for the first semester of the 2021-2022 school year. Requests for second semester will be made in November 2021. Requests to change a student's designation to in-person learning during the 2021-2022 school year due to changes in their circumstances may be done by contacting the Main Office where your student will be enrolled. Due to the health risk that would qualify a student for remote instruction, students approved for remote learning will not be allowed to participate in extracurricular activities.
